Care Nexus Memberships are personalized aging-at-home memberships that bring many of the services and amenities associated with assisted living into the member's own home—without changing where the member lives. Planning, care coordination, companion or caregiver support, hospitality services, wellness, lifestyle engagement, transportation, and family communication come together through one trusted advisor. Choose the level that fits today, then adjust as needs change.
Enrollment begins with a complimentary consultation and Lifestyle and Wellness Assessment. A Care Nexus Advisor recommends an Individual Service Plan based on the member's goals, routines, mobility, safety, social interests, cognition, and care needs. Services are coordinated through Care Nexus and vetted partners, subject to service area, availability, assessment, and the approved plan.
Senior living community selection and placement guidance is always free — Care Nexus Memberships memberships are separate paid plans for ongoing at-home coordination and included benefits.

Important: Connect is a coordination membership. Housekeeping, laundry, meals, transportation, event fees, companion care, hands-on caregiver services, and other partner services are purchased separately at member rates through the member account portal.

- Either one four-hour companion-service visit each week, or one four-hour hands-on caregiver visit each week for approved needs such as bathing assistance, escorting & more. Or weekly nursing visit for medication, injections, wound care or general wellness visits. Additional hours available at member rates.
- Personalized Care and Lifestyle Plan with quarterly review
- Two scheduled advisor contacts each month, including review of the member's Lifestyle and Wellness Assessment and recommendations from the calendar of activities, outings, and events
- Monthly update for an authorized family member or responsible party
- One housekeeping visit each week and one laundry pickup and return each week
- Thirty prepared-meals each month
- Four one-way local transportation credits each month, equivalent to two round trips

- Either two four-hour companion-service visits each week, or two four-hour hands-on caregiver visits each week for approved needs. Or weekly nursing visit for medication, injections, wound care or general wellness visits. Additional hours available at member rates.
- Weekly advisor or wellness check-in
- Monthly Care and Lifestyle Plan review
- Two housekeeping visits each week and one laundry pickup and return each week
- Sixty prepared-meals each month
- Monthly call with a registered dietitian, delivered by a qualified partner where required
- Eight one-way local transportation credits each month, equivalent to four round trips
- Monthly lifestyle review, premier registration priority, and two Lifestyle Privileges each month
- Hero Medication Dispenser setup and adherence check-ins within applicable law and partner scope
- Appointment preparation, follow-up, and authorized family summary

- A dedicated Care Nexus Advisor provides a weekly check-in and continuously reviews the member's Lifestyle and Wellness Assessment to recommend preferred or needed activities, outings, and events
- Multiple medical-alert-device options with fall monitoring, subject to device coverage, connectivity, vendor terms, and member consent
- A Care Nexus Advisor provides weekly planned family consultation and communication when requested and approved by the member
- Monthly plan review and reassessment after a material change or care transition
- Three housekeeping visits each week and two laundry pickups and drop-offs each week
- Ninety prepared meals each month, with members choosing the meals they prefer for breakfast, lunch, and dinner
- Ten one-way local transportation credits each month, equivalent to five round trips
- Ongoing lifestyle curation, highest registration priority, and four Lifestyle Privileges each month—generally one per week
- Hero Medication Dispenser setup and adherence check-ins within applicable law and partner scope
- Post-hospitalization transition coordination, appointment follow-up, and change-in-condition escalation

- If and when assisted living, a nursing home, or memory care is desired or needed, Care Nexus will develop a clear, supportive recommendation portfolio and coordinate tours, visits, and when available, a meal or activity experience.
Important: Enhanced is not a licensed assisted living residence, 24-hour supervision, emergency response, or unlimited care. Services are coordinated through qualified partners. Members who need daily multi-hour assistance require additional care hours or a custom plan. Delivery fees may apply to prepared meals.

Stay connected to the life you enjoy. Care Nexus curates meaningful programs, social gatherings, speakers, wellness experiences, and local excursions for members in each region. The Care Nexus Advisor highlights opportunities selected for the member, while the member or an authorized caregiver can browse the calendar and request registration through the member account portal at any time.
Lifestyle also includes Care Nexus Senior Adventures where a travel partner curates amazing adventures, from cruises to cross-country train trips.
